BORROWER LETTER

This letter is for all borrowers.  It explains the closing process and what documents and information we will need from you.  Click on this link and print out form.  Sign first page and return to our office. 

PAYOFF REQUEST

This form is for all borrowers and sellers.  Please print this form and fill in all information with the exception of the payoff date; our office will enter the necessary date.  It is important for each person on the account to sign the Payoff Request and include their social security number.  Then, return the completed form to our office via post, fax or hand-delivery. 

SURVEY AFFIDAVIT

This form is for sellers that our office will be representing at closing under power of attorney.  Please print and complete, then return to our office.

DECLARATION OF HOMESTEAD

This form must be signed and notarized.  Fill in the borrower's name (only one) on first line,  town goes in second line, full property address goes in third line.  You will X the appropriate box for how you obtained the property and fill in the recording information for that.  The rest is to be signed in front of a Notary Public.  Our office is pleased to offer this service free of charge to all of our clients.
